This expression comes from carpentry. When you hit a nail exactly on its head with a hammer, you drive it perfectly into the wood. Because of that, the phrase started to mean doing or saying something exactly right.
Interestingly, in Polish we have a very similar expression:
„trafić w sedno”, which literally means „to hit the core” of the problem.
